Choosing Your Primary Care Provider

Choosing Your Primary Care Provider

Edited December 24, 2024

As with all things new, change is involved. Dr. Taylor has been seeing the almost 1500 pts alone for 2 years. This is not sustainable and so we welcome Dr. Tracy Thomson. This means change for everyone and sometimes change can be hard. We ask that ALL patients choose a doctor by filling out the form below or a paper copy (available at the clinic) by Dec 16. Mid December was chosen as a deadline as we need time to update profiles in our EMR before Dr. Thomson's arrival. Providing patients the option to choose a primary care provider gives patients who might not be satisfied with their care an opportunity to choose a new provider ( Dr. Thomson). After we give consideration to preference the remaining patients will then be randomly assigned to meet each physicians patient quota. There is NO guarantee that you will get your first choice but we will do our very best.

In the past multiple physicians have worked to provide 1 full time position. Dr. Taylor will continue to work full-time and work Tues - Fri.  Dr. Thomson will work Mon-Wed. During vacations both doctors will cross cover each others pts for emergencies if a locum is not available. Some people have asked how can they decide as they don’t know Dr. Thomson yet? Please remember we are going to have 2 doctors for our community when there is a nationwide shortage. One way to decide is if you are happy with your care choose Dr. Taylor. If you are just somewhat happy and indifferent with your current care choose no preference. If you are not satisfied with your care choose Dr. Thomson. I hope this is clear but if unclear please call or visit the office or submit questions through the website.

Each adult in your household will need to complete their own form. A dependant is a child or adult who cannot make medical decisions for themselves. Please include your email address as well as your mobile phone number. Your email address and mobile number will be added to your profile in our EMR to allow you to book appointments online and receive appointment reminders. Please complete the form no later than December 16.

Thank you for submitting your primary care provider form. Electronic form submission is now closed.
